despite exporting settings file and re-importing after upgrade, all header information was missing. I had to remove:

Style & edit header=>configure header
%logo %pages %image

save

Same problem. The header was completely gone. Fixed by just saving in "Style & edit HEADER AREA"

do the same with the footer this was casued by the addition of two new features - Full width Header and Full width footer
