If you scroll down to the form, the names of the form fields are white (Jetpack form), so they are hidden. I can't work out where in Atahualpa settings I can change the colour of this text.

any clues?

Without looking over all your settings I don't know the "why" but you could use the following CSS to fix.

.grunion-field-label {
    color: black;
}
